FORMS

As part of the requirements of the Halton School Board and T.A.Blakelock High School, certain forms are required to be completed by students for participation in science classes. Please find the attached forms for your completion at the beginning of the term, as well as those to be completed when appropriate for the activities undertaken by the class.

Student Policy Review Survey

As part of a board wide review, certain staff members have been chosen to be part of a committee to review how the Halton Board applies the Growing Success document to its practices. I have been chosen to be a member of that committee, and I would appreciate it, if students would complete the following survey, focusing especially on the final 30 percent. Your feedback will help us to make changes to how we teach in the Halton Board, so please give us honest and serious feedback. If you have feedback about a specific teacher, please use the pseudonym "Mr. Davidson", so as to emphasize that it is not the board in general you are speaking about, but rather a specific experience.

Research Participant Forms

Students in senior sciences who undertake independent experiments must always do so under the guidance of the instructor. If their experiments require human research subjects, all subjects must sign a research release form. Attached is a skeleton of such a research release form, which may be altered to meet the specifics of the experiment and the experience the students are subjected to.

Field Trips

As part of the learning in our courses, students are given the opportunity, from time to time, to take part in field trips. Students will always be given field trip forms well in advance of an event, but in the event that a form is lost with an impending due date, forms can also be found here to be filled in.
